Miss J, You may want to contact Dr Philippon in Vail and see if he recommends anyone in Boston. I have friends who had this surgery with him. He is very well known for that surgery and people from all over the world go to him for that surgery. The recovery from it though, takes longer than a hip replacement; can take longer than a year. Alex Rodriquez a baseball player had FAI surgery. FAI correlates highly with young pro athletes but I'm a middle age woman.

have you had imaging? I have had two docs, a decade apart tell me they were sure I had a particular disorder in my foot structure (which would require surgery) from the symptoms and their physical examination... however, thank goodness I had an MRI because there was nothing there. the first doc was ready to cut me open and I insisted on the MRI. I know you are on the way to finding a physician and that's good... they will do some kind of films. The Hospital for Special Surgery in new york is teeming with all kinds of joint specialists... if you can't find someone directly in the boston area, you might try HSS and see who is affiliated with them in boston.

Robin is absolutely correct, and the only way to diagnos FAI is to get an MRI arthrogram not a regular MRI. You need to have the dye injected directly into the hip joint. Robin wrote:

Robin, I have not has the type of MRI imaging to confirm the diagnosis. However, I would like a doc conversant in FAI or at least with a focus on JOINT PRESERVATION to confirm the diagnosis. Technically/legally speaking what I have might not be considered an 'official' diagnosis as my physical therapist strongly suspects I have FAI because I tested positive for all the diagnostic PT tests they do for that and also, according to an article in "American Family Physician", my complaints are right on the target for FAI. When I went to the physical therapist, I had already done a lot of research as to what possibly could be wrong in addition to the arthritis they saw. The list was stuff ' I hope I don't have' such as A vascular necrosis, occult femoral hernia, femoral (or other nerve) IMPINGEMENT and FAI as I was HOPING it was just a groin muscle pull. So, some of those things were ruled out but I tested positive for the FAI. Now the PT did not tell me I needed surgery and we were both hoping that the PT could help. I'm pretty confident that her findings were correct. I'm confident they are correct because the cross reference with all my research about FAI (which I was hoping I did not have). Don't really have the resources to go out of state for this although some NYC docs conversant in FAI did come up in some of my searches. Yes, an MRI is needed to confirm the diagnosis. I'm just searching around for docs who have a focus on joint preservation via small scar techniques (arthroscopy) who are experienced treating FAI. Like those are the ones I wish to consult with as I have to narrow down my consults. I know what you mean about insisting on an MRI. I too want one but in reference to FAI. I actually do have X rays and the PT could SEE the morphology that conformed to FAI in the X rays. However, the X rays were taken at Mt Auburn where the doc looking at them was just trying to rule out FRACTURE. Also, since FAI is a relatively 'new' diagnosis in orthopedics, it's not something an emergency doc reading an X ray is really going to look for. Believe me, I'm NOT looking for surgery and hesitate to go to an orthopedic doc who's 'default' is hip replacement. Those are the docs I want to first rule out. Robin wrote:

Well, yes that is true. I would be looking to confirm a diagnosis of FAI that way but would need to find a doc who had as his OBJECTIVE to either confirm or rule out FAI--like not just some MRI to take a general look. I’m not going to try and pretend like physicians are always professional. I mean, especially in surgery. It’s a bunch of guys—mostly, still. It’s not a “hot or not” conversation because, I mean, really, what percentage of patients are actually hot? But it’s “hotter,” “better,” you know? Yes, all that's totally true. I live with a surgeon (not a plastic surgeon though) and I was around when he was in training. You'd flip, totally flip, if you heard some of the stories I've heard. Flip.
