If you look at early pics of Madonna, it does not appear she ever needed (or had) any implants. The look of the chin that happens with pursed lips exaggerates the chin (see below). Unfortunately, Madonna tends to get caught in pics doing this a lot, and it's not very flattering.

Sure, but the chin looks larger even without doing that. No, she never needed it, but as we age, even if we have a chin, doctors tend to put an implant in to take up slack....or filler.

Not in all cases, i didn't have any slack when they pit the chin implant, the Dr that put it in didn't notice my chin deficiency during consultation, he noticed later on until he saw my pictures ( I blame it on the way the pictures are taken by office photographer.)
